Medical Release

I agree to the medical release
We will notify a parent/guardian when a participant is ill or needs medical attention. In the event we cannot contact you and we need to get immediate help for your child, we require consent to do so.
1. I give consent for my child to be taken to the nearest emergency medical centre by ambulance when I cannot be contacted.
2. I give consent for my child to receive first aid treatment only by those certified to do so.

If your child is participating in our programs and their behaviour is a safety issue, or is behaving in a way that prohibits the learning of others in the program, you acknowledge that you may be required to pick up your child early from the program. In the event we are unable to get a hold of you, you consent to your Emergency Contact (as indicated on this form) being contacted for pickup. While we make every effort to work with families and behaviors, you acknowledge that your child may be dismissed for the remainder of the program depending on the severity of the behaviour.

Payment:
Payment is due at the time of registration. To pay by cheque, please e-mail us at wild.immersive.wl@ubc.ca
Cancellation Policy:
A lot of preparation and effort goes into planning our events. Registrations can be cancelled with a minimum of 1-month notice and a refund will be issued less a $25 administration fee. Cancellations with a minimum of 14 days’ notice will not receive a refund. In the event of illness, the student can have their registration transferred to another participant, or credited towards a future program. If Wild & Immersive has to cancel, a full refund will be issued.
Illness Policy:
There is zero tolerance for any child attending camp while sick or exhibiting cold or flu like symptoms. If your child is ill, do not send them to camp. If your child develops symptoms at camp, they will be separated from the group and sent home.
What to Bring:
Please be prepared! Events are outdoors and run in all weather conditions (with the exception of safety concerns). Participants must have appropriate clothing and footwear for all weather conditions.
Lunch is not provided, please ensure your child has lunch, healthy snacks, and water for the day.
Participation and Behaviour:
The safety of each individual at Wild & Immersive is of the utmost importance. Any behaviours that place the student or others at risk may result in the students immediate dismissal from the program. If dismissed, the parent/guardian accepts costs for the remainder of the program. Refunds will not be granted to students who refuse to participate in the program.
